Are they built into the game, or multiplayer custom items?

It's in the game, but it's hard work to get them. you have to battle about 20 or so level 40 Prarie Dogs (they repeatedly knocked out my combat mage) and insanely powerful level 40 copes of your party members. I rather did laugh out heartily at some of the things the dev team said. There's even a zork-esque mini-game...
