Actually, the first non capitalised B is a continuation of the first line, which ends in a comma. It just has a few carriage returns in it, as is normal in the beginning of a letter.
Don't know what's official, but as far as I'm aware a carriage return isn't any form of punctuation and so should probably be ignored. I suspect it stems from the fact that people are confusing the strart of a new line with the start of a new sentence.
I think we need eion's advice on this one, as I have no idea! I've always started letters as above, and Word auto-corrects to a capital after the salutation if you go to a new line, but then I wouldn't put too much stock in that.
The start of the line below the addressee greeting should be capitalised. The comma in this case has a slightly different meaning than used in a proper sentence.
